The Emmetsburg Community School District will be working with Palo Alto County Community Health to offer H1N1 vaccinations to our students. We sent out information today containing a parent information letter, consent form, and H1N1 information sheet. If you would like your child to receive this vaccination, please complete the consent form and return it to the school office by Monday, November 23rd. The vaccinations will likely take place on Tuesday or Wednesday of next week at school. The nasal vaccine will not be available. If you have any questions on the process, please contact a school office. If you have any questions on the vaccine, please contact your health care provider. Below are additional links to the forms.
